Carriage of the allele was\u00a0identified\u00a0in 2,009 patients (4.6%) and showed consistent links with more aggressive disease behaviour.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h2><b><i><span data-contrast=\"auto\">HLA-DRB1*01:03\u00a0<\/span><\/i><\/b><b><span data-contrast=\"auto\">and Severe IBD Outcomes<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"auto\">In Crohn\u2019s disease,\u00a0<\/span><i><span data-contrast=\"auto\">HLA-DRB1*01:03\u00a0<\/span><\/i><span data-contrast=\"auto\">was associated with higher odds of colonic resection (OR: 1.35; 95% CI: 1.07\u20131.69), perianal disease (OR: 1.65; 95% CI: 1.42\u20131.92) and increased use of advanced therapy (OR: 1.33; 95% CI: 1.12\u20131.58). In ulcerative colitis or\u00a0IBD\u00a0unclassified, the allele showed stronger associations with colectomy (OR: 1.99; 95% CI: 1.63\u20132.43), perianal disease (OR: 1.70; 95% CI: 1.27\u20132.28) and advanced therapy use (OR: 2.17; 95% CI: 1.86\u20132.52).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The allele was also associated with differences in disease onset patterns, being linked to younger onset in ulcerative colitis or\u00a0IBD\u00a0unclassified and older onset in Crohn\u2019s disease.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h2><b><span data-contrast=\"auto\">Earlier Progression\u00a0and\u00a0Escalation\u00a0of Therapy<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"auto\">Time-to-event analyses\u00a0demonstrated\u00a0that\u00a0<\/span><i><span data-contrast=\"auto\">HLA-DRB1*01:03\u00a0<\/span><\/i><span data-contrast=\"auto\">carriers experienced earlier disease complications. In Crohn\u2019s disease, the allele was associated with earlier development of perianal disease (HR: 1.61; 95% CI: 1.24\u20132.07) and earlier need for colonic surgery (HR: 1.43; 95% CI: 1.13\u20131.82). In ulcerative colitis or\u00a0IBD\u00a0unclassified, carriers underwent colectomy earlier (HR: 1.69; 95% CI: 1.33\u20132.14).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Treatment escalation followed a similar pattern, with earlier initiation of advanced therapies in Crohn\u2019s disease (HR: 1.37; 95% CI: 1.18\u20131.59) and ulcerative colitis or\u00a0IBD\u00a0unclassified (HR: 1.82; 95% CI: 1.53\u20132.16). Carriers also had a higher risk of advanced therapy failure across\u00a0IBD\u00a0phenotypes (HR: 1.23; 95% CI: 1.02\u20131.50).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h2><b><span data-contrast=\"auto\">Clinical Implications\u00a0<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"auto\">The findings support\u00a0<\/span><i><span data-contrast=\"auto\">HLA-DRB1*01:03\u00a0<\/span><\/i><span data-contrast=\"auto\">as a genetic marker of severe\u00a0IBD\u00a0outcomes\u00a0across phenotypes rather than disease susceptibility alone.
