Patients with central\u00a0RAO\u00a0experienced\u00a0substantially\u00a0increased\u00a0risk of both\u00a0<\/span><a href=\"https:\/\/www.emjreviews.com\/neurology\/news\/novel-mri-boosts-detection-of-stroke-causing\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">ischaemic stroke<\/span><\/a><span data-contrast=\"auto\">\u00a0and\u00a0<\/span><a href=\"https:\/\/www.emjreviews.com\/cardiology\/news\/heart-failure-predicted-up-to-five-years-early\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">myocardial infarction<\/span><\/a><span data-contrast=\"auto\">,\u00a0compared\u00a0to their\u00a0matched controls.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h2><b><span data-contrast=\"auto\">Retinal Artery Occlusion and Cardiovascular Outcomes<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"auto\">The study included 566 patients diagnosed between 2011 and 2018 with\u00a0acute\u00a0retinal\u00a0arterial ischaemia (either\u00a0central\u00a0RAO, branch\u00a0RAO, or transient\u00a0vision\u00a0loss)\u00a0at\u00a0Moorfields Eye Hospital<\/span><span data-contrast=\"auto\">,\u00a0<\/span><span data-contrast=\"auto\">London, United Kingdom.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Among 190 patients with central\u00a0RAO, five developed myocardial infarction and seven experienced ischaemic stroke following diagnosis. Compared with propensity matched controls, central\u00a0RAO\u00a0was associated with an increased risk of myocardial infarction (p=0.047) and ischaemic stroke (p=0.014). Mortality risk was also\u00a0observed\u00a0to be\u00a0elevated,\u00a0at the threshold of statistical significance (p=0.05).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h2><b><span data-contrast=\"auto\">Atrial Fibrillation Detection Highlights Ongoing Risk<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"auto\">The data also\u00a0highlighted\u00a0the\u00a0substantial burden of cardiovascular comorbidity.\u00a0High blood pressure, diabetes, and\u00a0elevated\u00a0cholesterol\u00a0levels\u00a0were common across all groups. In patients with central\u00a0RAO, 6% received a new diagnosis of atrial fibrillation within\u00a0one\u00a0year of the event.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Investigators\u00a0proposed\u00a0that atrial fibrillation may\u00a0represent\u00a0an important contributor to retinal ischaemic events. The findings support consideration of prolonged cardiac monitoring following acute retinal ischaemia, particularly in patients with central\u00a0RAO.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h2><b><span data-contrast=\"auto\">Differences Between Retinal Ischaemic Presentations<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"auto\">In contrast to central\u00a0RAO, neither branch\u00a0ROA\u00a0or\u00a0transient\u00a0vision\u00a0loss showed significant increases in myocardial infarction, ischaemic stroke, or mortality compared with matched controls.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">However, approximately 5% of patients with branch\u00a0RAO\u00a0or transient\u00a0vision\u00a0loss underwent carotid endarterectomy following their event. Patients with branch\u00a0RAO\u00a0were\u00a0also\u00a0significantly more likely to receive carotid endarterectomy than those with central\u00a0RAO\u00a0(p=0.04).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The\u00a0researchers\u00a0concluded that the cardiovascular burden associated with\u00a0RAO\u00a0extends beyond acute\u00a0vision\u00a0loss.
