post-stroke cognitive decline, according to new evidence from\u00a0Seoul, Republic of\u00a0Korea.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The findings come from a retrospective\u00a0population-based\u00a0study of 359,572 individuals who underwent repeated standardised cognitive assessments.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Researchers examined\u00a0data\u00a0from the Korean National Health Insurance Service and Long-Term Care Insurance databases.\u00a0Changes in cognitive function\u00a0were evaluated\u00a0using repeated cognitive function equivalent score assessments.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Participants were categorised according to\u00a0<\/span><a href=\"https:\/\/www.emjreviews.com\/nephrology\/news\/survival-risks-in-chronic-kidney-disease\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">chronic kidney disease<\/span><\/a><span data-contrast=\"auto\">\u00a0(CKD)\u00a0status and stroke timing, allowing investigators to assess whether kidney disease\u00a0significantly\u00a0affected\u00a0the trajectory of cognitive decline after\u00a0<\/span><a href=\"https:\/\/www.emjreviews.com\/cardiology\/news\/microplastics-linked-to-stroke-and-cardiometabolic-disease\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">stroke<\/span><\/a><span data-contrast=\"auto\">.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h2><b><span data-contrast=\"auto\">Stroke Survivors\u00a0with\u00a0CKD\u00a0Face Faster Cognitive Decline<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"auto\">CKD\u00a0alone was associated with a\u00a0moderately\u00a0faster rate of cognitive decline compared with individuals without\u00a0CKD\u00a0or stroke.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Pre-existing\u00a0stroke alone was not significantly associated with accelerated decline, but when\u00a0CKD\u00a0co-existed with\u00a0pre-existing\u00a0stroke, the association increased substantially.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Incident stroke was also linked to faster deterioration in cognitive function.\u00a0The effect was numerically greater among individuals with both chronic kidney disease and incident stroke.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The largest effect size was\u00a0observed\u00a0in participants with recurrent stroke and\u00a0CKD.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h2><b><span data-contrast=\"auto\">Evidence of a Stroke-Specific Effect<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"auto\">A significant positive interaction was\u00a0identified\u00a0between\u00a0CKD\u00a0and\u00a0pre-existing\u00a0stroke on the continuous cognitive outcome scale.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Interactions involving incident and recurrent stroke showed similar directional trends but did not achieve statistical significance.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">When clinically significant cognitive decline was assessed, the combination of\u00a0CKD\u00a0and incident stroke produced the highest risk, exceeding the risk associated with incident stroke alone.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<h2><b><span data-contrast=\"auto\">Findings Not Replicated in Neurodegenerative Disease<\/span><\/b><span data-ccp-props=\"{}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"auto\">Secondary analyses explored whether\u00a0CKD\u00a0similarly amplified cognitive decline in dementia and Parkinson\u2019s disease.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">No consistent acceleration of cognitive decline was\u00a0observed\u00a0when\u00a0CKD\u00a0co-occurred with these neurodegenerative conditions.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Interaction estimates were either null or negative, suggesting the observed association may be more specific to vascular disease processes than to neurodegeneration.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">While evidence for interaction varied across analytical approaches, the findings\u00a0indicate\u00a0that individuals with\u00a0CKD\u00a0who experience stroke may\u00a0represent\u00a0a population at increased risk of accelerated cognitive deterioration and may\u00a0warrant\u00a0closer cognitive
