Optimising intraoperative ventilation has become an important strategy for reducing lung injury during surgery. PCV-VG combines the pressure-limiting benefits of pressure-controlled ventilation with the delivery of a guaranteed tidal volume, potentially improving lung protection while maintaining adequate ventilation.<\/p>\n<h2 data-section-id=\"1vhaa93\" data-start=\"1571\" data-end=\"1616\">Trial Compared Two Ventilation Strategies<\/h2>\n<p data-start=\"1618\" data-end=\"1800\">Researchers conducted a single-centre randomized controlled trial involving 106 adults undergoing emergency laparotomy expected to last more than two hours under general anaesthesia.<\/p>\n<p data-start=\"1802\" data-end=\"2085\">Participants were randomly assigned to receive either PCV-VG or conventional volume control ventilation throughout surgery. The primary outcome was the development of postoperative pulmonary complications within seven days, assessed using the Melbourne Group Score version 2 (MGS-2).<\/p>\n<p data-start=\"2087\" data-end=\"2261\">Secondary outcomes included respiratory mechanics during surgery, lung aeration assessed by ultrasound, oxygenation, hospital length of stay, oxygen-free days, and mortality.<\/p>\n<h2 data-section-id=\"1dtfujn\" data-start=\"2263\" data-end=\"2310\">Better Respiratory Mechanics During Surgery<\/h2>\n<p data-start=\"2312\" data-end=\"2481\">One hour after surgery began, patients receiving PCV-VG demonstrated significantly improved respiratory mechanics compared with those receiving conventional ventilation.<\/p>\n<p data-start=\"2483\" data-end=\"2665\">Respiratory system compliance was higher, while both driving pressure and mechanical power\u2014two measures associated with ventilator-induced lung injury\u2014were lower in the PCV-VG group.<\/p>\n<p data-start=\"2667\" data-end=\"2845\">These findings suggest the ventilation mode may reduce mechanical stress on the lungs during surgery by delivering tidal volumes more efficiently while limiting airway pressures.<\/p>\n<p data-start=\"2847\" data-end=\"2929\">However, oxygenation and lung aeration scores were similar between the two groups.<\/p>\n<h2 data-section-id=\"1omdk71\" data-start=\"2931\" data-end=\"2968\">Clinical Outcomes Were Comparable<\/h2>\n<p data-start=\"2970\" data-end=\"3107\">Despite the physiological improvements observed during surgery, the primary clinical outcome did not differ significantly between groups.<\/p>\n<p data-start=\"3109\" data-end=\"3266\">Median postoperative pulmonary complication scores were comparable, and statistical analysis found no significant reduction in overall PPC rates with PCV-VG.<\/p>\n<p data-start=\"3268\" data-end=\"3599\">Using the Melbourne Group Score definition, postoperative pulmonary complications occurred in 3.8% of patients receiving PCV-VG compared with 17% of those receiving volume control ventilation. Although this numerical difference favoured PCV-VG, the study was not powered to determine whether this reflected a true clinical benefit.<\/p>\n<p data-start=\"3601\" data-end=\"3734\">Other patient-centred outcomes, including oxygen-free days, hospital length of stay, and mortality, were also similar between groups.<\/p>\n<h2 data-section-id=\"4v1w5n\" data-start=\"3736\" data-end=\"3761\">Larger Studies Needed<\/h2>\n<p data-start=\"3763\" data-end=\"3960\">The authors conclude that PCV-VG offers measurable physiological advantages during emergency laparotomy by improving respiratory system mechanics and reducing driving pressure and mechanical power.<\/p>\n<p data-start=\"3962\" data-end=\"4240\">However, these improvements did not translate into a statistically significant reduction in postoperative pulmonary complications in this study. The researchers note that the relatively small sample size may have limited their ability to detect differences in clinical outcomes.<\/p>\n<p data-start=\"4242\" data-end=\"4473\" data-is-last-node=\"\" data-is-only-node=\"\">They suggest that larger multicentre trials are now needed to determine whether the intraoperative benefits of PCV-VG ultimately improve postoperative recovery and reduce pulmonary complications in higher-risk surgical populations.<\/p>\n<p data-start=\"4242\" data-end=\"4473\" data-is-last-node=\"\" data-is-only-node=\"\"><strong>Reference<\/strong><\/p>\n<p data-start=\"4242\" data-end=\"4473\" data-is-last-node=\"\" data-is-only-node=\"\">Jassal N et al.
