Gastrointestinal (GI) disorders such as acidity, dyspepsia, and irritable bowel syndrome (IBS) are among the most common reasons for outpatient consultations. While acidity is often attributed to excess gastric acid, IBS represents a functional disorder of gut\u2013brain interaction, highlighting the complex interplay between gastrointestinal physiology and psychological health.<\/p>\n

IBS affects nearly 10\u201320% of the Indian population and is characterized by recurrent abdominal pain associated with altered bowel habits, including constipation, diarrhea, or a combination of both. According to the Rome IV criteria, IBS is classified into constipation-predominant (IBS-C), diarrhea-predominant (IBS-D), mixed (IBS-M), and unclassified subtypes. Importantly, IBS is a clinical diagnosis based on symptom patterns rather than a diagnosis of exclusion.<\/p>\n

A thorough clinical assessment remains the cornerstone of diagnosis. Alarm features\u2014including symptom onset after 50 years of age, unintended weight loss, gastrointestinal bleeding, anemia, nocturnal diarrhea, persistent fever, or a family history of colorectal malignancy or inflammatory bowel disease\u2014should prompt further investigations to exclude organic pathology.<\/p>\n

Management extends well beyond symptom relief. Dietary modification, particularly a low-FODMAP diet in selected patients, regular physical activity, adequate hydration, and stress management constitute the foundation of care. Symptom-directed therapies, including soluble fiber, antispasmodics, probiotics, peppermint oil, and pharmacological agents tailored to IBS subtype, can provide meaningful relief. Equally important is addressing the psychological dimension of the disease. Anxiety, stress, and depression frequently coexist with IBS, making patient education and, when appropriate, cognitive behavioral therapy integral components of treatment.<\/p>\n

As our understanding of the gut\u2013brain axis continues to evolve, so too must our approach to common gastrointestinal disorders. Moving beyond the simplistic notion of \u201cacidity\u201d toward a comprehensive, patient-centered evaluation enables clinicians to deliver more accurate diagnoses, personalized treatment, and sustained improvements in patient well-being. Early recognition, reassurance, and multidisciplinary care remain the key to optimizing outcomes in functional GI disorders.<\/p>\n
